A clinical study on the choice of etching agent for pit and fissure sealing in younger children

Abstract: Objective Compare the retention rate of fissure sealants when using self-etching adhesive and phosphate acid for the clean tooth surfaces of young children. To evaluate the effect of the application of pit and fissure sealant with self-etching adhesives and phosphoric acid. Methods Sixty-seven children with complete eruption and no caries of the four second primary molars were selected as the study subjects. Totally 268 teeth of the second primary molars were used as test teeth, divided into four groups with the method of self control test. The right Group A(maxillary) and B(mandibular) were treated with self-etching adhesive, and the left Group C(maxillary) and D(mandibular) with phosphate-acid etching method. All the children were followed up and the rate of shedding of the four groups was compared after 3, 6, 12 and 24 months. Results The retention rates of pit and fissure sealant in four groups(A~D) had no significant difference after 3 and 6 months(P>0.05). At the 12 months, there was significant difference between the A, C group and the D group(P<0.05), but there was no significant difference between the B, D group and the A, B, C group respectively(P>0.05). At the 24 months, the retention rate of pit and fissure sealant in Group D was statistically lower than in the A, B, C group(P<0.05), but the other three groups(A, B, C) had no significant difference in the retention rate of pit and fissure sealant(P>0.05). The retention rate of the mandibular group with phosphoric acid etching was significantly lower than the other three groups. Conclusion The mandibular molars are more susceptible to saliva contamination. Self-etching adhesive can reduce the influence of saliva pollution and improve retention rate of fissure sealing. It is recommended to be used for young children in clinical.
