    Abstract ( 1096 )   HTML ( 4 )   PDF(pc) (1116KB) ( 767 )   Save

Objective Based on a periosteal distraction to periosteum of cleft palate edge, to observe the structural features of newly formed bone in each phase, study its forming process and rule, and make a preliminary study on the mechanism of repairing cleft palate. Methods Firstly, develop model of cleft palate in 8 healthy hybrid juvenile dogs. Then place Nitinol periosteal distraction device under the periosteum of cleft palate edge and fix it and distract the periosteum with a force of(2.45±0.20)N. The opposite edge belongs to control group. Every 2 dogs were killed after 20, 30, 40, 60 days respectively. Results New bone eventually generated under the periosteum after the periosteal distraction at the edge of cleft. Histological observation indicates the increase of osteoblasts in the new bone and the changes of the directions of the collagenous fiber and bone trabecula. A large amount of collagenous fibers arrayed toward the direction of distraction, under the influence of proliferation of osteoblasts, was calcified and become new bone, which gradually became more mature. Conclusion Newly formed bone is completed along the distracting direction in the periosteum. New bone generates in the distraction zone 20 days after the implantation of periosteal distraction device. With the time of placement extending, bone grows, which is most vigorous on the 40th day and becomes relatively mature in terms of calcification on the 60th day focusing on transformation and modeling.

Objective To evaluate the interfacial stress distribution in different class Ⅳ preparation designs with composite resin restoration at centric occlusion and protrusive occlusion. Methods Three dimensional finite element model of maxillary central incisor with class Ⅳ preparation was established by spiral CT. It was designed into 1 mm and 2 mm bevels, chamfer, stair-step chamfer preparation and untreated model as control. All the models were restored with composite resin, and loaded under 100 N forming 45° with long axis. The interfacial Von Mises stress and distribution was recorded. Results The maximum Von Mises stress values of the interface at centric occlusion were obviously smaller than that of protrusive occlusion, at which the stess level of two bevels were smaller than two chamfers. Nevertheless, the distribution of stresses is more uniform in chamfers than that in bevels. Conclusion In view of stress and its distribution, the bevel and chamfer both have their superiorities. The maximum Von Mises stress values of the interface at protrusive occlusion were obviously higher than that of centric occlusion, thus class Ⅳ prepared teeth restored with composite resins should avoid biting hard food edge to edge.

Objective To investigate the effect of Chinese tradition medicine on mandibular distraction osteogenesis. Methods Sixteen beagles were randomly divided into medicine group and control group. A customized component distraction device was applied to the right side of mandible, and the transverse osteotomy was performed in all animals. After 7 days, distraction was started at the rate of 0.5 mm each 12 h and continued for 10 days. The beagles in medicine group drank Chinese tradition medicine from the first day after postoperation to the last of the trial. 2 animals were killed at week 1, 2, 3, and 4 after consolidation. The mandibles were harvested to examine the distraction tissues histologically, bone histomorphontry measures and detect of osteocalcin(OC). Results 1)Observation of the general specimens: During the 1-2 weeks of the fixed period, no significant difference was shown between the two groups. The distraction gap was covered with a thick layer of collagen connective tissue, and there was a slight movement of distraction gap. In the 3 weeks, from the two group samples, the proliferated periosteal get thinner, and the movement of two bone stumps disappeare could be observed. After the 4 weeks, the thickness of the periosteum at distraction gap of the two groups gradually returned to normal. It was difficult to identify the two-cut stump of the bone. New bone tissue of the medicine group was better than that of the control group, and bone tissue at the distraction gap was more dense and smooth. 2)Histomorphological changes:In the 1st and the 2 weeks of the fixed period, there was no significant difference between the two groups. In the 1st week, a small part of distraction zone was filled with honeycomb connective tissue, and close to the nearly cut of marrow department, new membranous bone formed. In the 2 weeks, the distraction gap from the center to both sides, showed three clear bands: The fiber zone, finger osteoid zone and newborn trabecular zone. In the 3 weeks, new bone formation can be seen in most of the distraction regions. In the medicine group, new bone formation within the distraction gap was active, new bone trabecular was thick, and new bone tissue was also more mature. In the 4 weeks, the three typical bands at distraction gap were disappeared, replaced by the new parallelly ranked trabecular bone. In the medicine group, bone trabecular ordered densely, thick and uniform in shape, while the control group, bone trabecular was sparse and uneven thickness. 3) Bone histomorphometry analysis:In the 1st and the 2 weeks of the fixed period, trabecular bone’s area, trabecular bone’s perimeter, and trabecular bone surface’s density had no statistically significant difference between two groups. In the 3-4 weeks, in the medicine group trabecular area, trabecular perimeter and area density were significantly higher than those of the control group. 4)The detect of OC:Oc mRNA was mainly expressed on osteoblast and the surfaces of new bones. In the 1st week of the fixed period, the expression of oc mRNA had no statistically significant difference between the two groups. In the 2, 3, 4 weeks of the fixed period, the expression of oc mRNA of medicine group was significantly higher than that of the control group. Conclusion Chinese tradition medicine can effectively accelerate the formation and maturity of new bone of mandibular distraction through increasing osteogenic gene expression.

Viral introduction of four genes(oct-4, sox-2, c-myc, klf-4) in committed somatic cells is sufficient to yield induced pluripotent stem(iPS) cells that resemble embryonic stem cells. Since dental pulp stem cell (DPSC)can be easily obtained from extracted teeth, attempts to reprogram DPSC to iPS cells by defined pluripotent transcription factors may provide a more advantageous seed cell source for tooth regeneration. This review mainly covers the structures and function of the defined pluripotent transcription factors, the roles they play in de-differentiation of somatic cell and in tumor genesis, and their expressions and significances in dental pulp cell.

Although tissue engineering has been thought of having great potential in organ regeneration, there are still some bottlenecks, such as immune rejection of the scaffold materials, inflammation caused by fibrosis as well as cell proliferation within the stent material. Cell sheet transplantation, by which cultured cells can be harvested as intact sheets, has much superiority over traditional tissue engineering. The cell sheet, with no need of scaffold, can be transplanted by single piece or forming three-dimensional structure through same or a different cell sheet stack with each other. It avoids the complication of scaffold material degradation and will have a prosperous application in the future.

It is significantly important to distinguish the pulpal status in determining plans of odontal prosthesis and endodontic therapy. There are pulp sensibility testing and pulp vitality testing currently. The sensibility testing is based on patients’feeling by stimulating the nerve in the pulp, which is subjective and cannot reflect the real status of the pulp. However, the pulp vitality testing can reflect the pulpal vitality by detecting its blood supply, simultaneously being objective and non-invasive, which makes its advantages in clinical application. This review discussed the difference between the pulp sensibility testing and pulp vitality testing, as well as recent progresses in the field of pulp vitality test.

The ability of bacteria to sense and respond to population density is termed cell-to-cell communication or quorum sensing system(QSS). And many bacterial physiological functions such as biofilm formation and virulence are regulated by QSS. QSS is seemed to be a novel target for some antimicrobial drugs which can block reception of the cognate QSS signals in vivo and finally inducing chemical attenuation of pathogens. Recently, some researchers have found that some natural substances can interfere with the cycle of QSS. The natural inhibitors of QSS will be introduced in this view.
