    Failure factors and prevention strategy of root canal therapy

    Root canal therapy is the efficient treatment of dental pulp diseases and root periodontal diseases. There are many factors which affect the treating effect such as doctor, patient and other related areas. Now we analysis these factors and give advices to help dentists to elevate the success rate of root canal therapy.

    Objective To investigate the expression of CXC receptor(CXCR) 4 and its role in oral squamous cell carcinoma(OSCC). Methods SP immunohistochemical method was used to detected the expression of CXCR4 in OSCC. Results There were positive correlations between the expression of CXCR4 and the differentiation, TNM classification, lymph node metastasis of tumor. Conclusion CXCR4 may play a key role in the emergence and development of OSCC.

    Objective To fabricate a surgical template to solve the problem of the precise positioning and orientation in the process of dental implant. Methods Following a comprehensive oral examination and precise measurement by CT and Photoshop, the shape of the basal alveolar bone of the planed implant area was transferred onto the plaster sectional model. Then the site and orientation of the fixtures could be designed in the range of dentoalveolar and matrix bone and positioning template could be made. With this template, 27 implant caves of one pair skeletal and mandibular bone were prepared. Results A satisfied result was achieved by using t paired test. This template could enhance the accuracy of placement in an efficient way to achieve predictable esthetic results. Conclusion The underlying basal alveolar bone can be used most effectively with this template. Also, it can extend the precision of dental implantation. This method showed better clinical treatment effects and it is suitable for widespread use in clinical practice.

    Preliminary study of screening oral bacteria producing hydrogen sulfide by cotton balls with lead acetate

    Objective To screen common oral bacteria which produce hydrogen sulfide, and expect to find the main source of bad breath. To analyze the relationship between halitosis and oral caries, periodontal diseases. Methods Based on the color reaction between hydrogen sulfide and lead acetate, we use indirect experimental method by cotton-wool to detect 8 bacteria as Streptococcus, Actinomyces, Lactobacillas which are about caries and 9 bacteria as Porphyromonas, Prevotella, Fusobacterium which are about periodontal to identify the ability to produce hydrogen sulfide. Results All of the 9 bacteria about periodontal disease can produce H2S. In the bacteria group about caries, only Streptococcus gordonii, Actinomyces naeslundii, Actinomyces odontolyticus, Lactobacillus acidophilus can produce H2S while Streptococcus mutans, Streptococcus sobrinus, Actinomyces viscosus, Lactobacillus fermentatae can not. Conclusion Most of the bacteria which can produce H2S are gram-negative anaerobic bacteria. The bacteria that produce halitosis have relationship with the occurrence and development of periodontal disease, pulposis. And almost one half of the cariogenic bacteria can produce hydrogen sulfide, which can result halitosis.

    Objective The objective of this study was to evaluate the effects of three home application desensitizers( Sensodyne, Colgate and Xinxiaogan) on the occlusion of the dentinal tubules in vitro. Methods Dentin discs were used to prepare the dentin hypersensitivity model. Specimens were then subjected to brush experiment using different desensitizers with the common dentifrice serving as a control. Morphology of specimens in different groups before and after treatment were observed under scanning electron microscope(SEM), meanwhile, active ingredients on the dentine surface were analyzed by energy disperse spectroscopy(EDS). The degree of occlusion of the dentinal tubules was quantified using an image analyser. Results Distinct surface coatings were observed by SEM in vertical and horizontal plane of dentin. The use of all desensitizers occluded tubules more than no brushing and brushing with ordinary dentifrice(P<0.05). There was no significant difference between all the experimental groups(P>0.05). In the clogging matter, EDS revealed the presence of active ingredients of F. Conclusion The tested dentifrices seem effective for desensitization by tubule occlusion.

    Objective To explore the relationship between nonsyndromic cleft lip with or without cleft palate (NSCL/P) and genetic polymorphism of muscle segment homeobox gene(msx)-1 A448T in Shanxi province population. Methods Case-control study design was employed. msx-1 genotypes were determined by polymerase chain reaction and restriction fragment length polymorphism techniques. Results It was obvious difference between NSCL/P group and control group in msx-1 A448T mutation allele(T) frequency(P<0.01). The frequency of allele (T) of control group was higher than NSCL/P group(0.55>0.26). Conclusion We could confirm a significant association between msx-1/A448T polymorphism and NSCL/P in Shanxi province, China. T allele has protective effect in NSCL/P occurrence.

    Objective The study was designed to evaluate the comparison and correlation of soft and hard tissue profile changes in the extraction treatment using maximum anchorage in adolescent and adult cases. Methods Comparing 9 indicators about soft and hard tissues of those 13 adolescent and 14 adult cases between pre- and post- treatment with t-test respectively. After that, select some indicators and study the correlation of them. At last, a regression was deployed in this research. Results This study showed that: 1)In the adolescent group, U1- SN, U1-PM, the degree of crowding were changed significantly after the treatment. The decrease of the upper incisors and upper lip protrusion negatively correlated with the degree of crowding before the treatment, and positively correlated with the distance between the upper lip to E-line before the treatment. The linear regression equation between upper incisors and lip was y=-2.900+0.926x(y, changes of upper lip; x, changes of upper incisors). 2)In the adult group, all of those indicators were significantly changed except SNA and Ls -PM. The decrease of the upper incisors positively correlated with the degree of incisor inclination on the lip before the treatment, and negatively correlated with the degree of crowding before the treatment. However, the decrease of the upper lip protrusion positively correlated with the degree of incisor inclination on the lip before the treatment. The linear regression equation was y =-1.027 +0.706x(y, changes of upper lip; x, changes of upper incisors). Conclusion Extraction treatment with maximum anchorage has a good effect on correcting crowding and drawing back the position of upper incisors as well as upper lip. The decrease of upper incisors and lip protrusion is mainly influenced by the quantity of upper incisors′ protrusion and crowding in adults. But for adolescents, it will be complex, maybe growth is one of the important causes.

    Application of auxiliary pole in esthetic prosthodontic treatment of posterior residual root

    Objective To evaluate the treatment outcome of the auxiliary pole in anterior teeth restorations. Methods 98 anterior teeth were chose from 51 patients. They were divided into two groups. Experimental group were composed of 56 teeth. Control group were composed of 42 teeth. The teeth of experimental group were restored using the D.T.Light-Post and auxiliary pole. The teeth of control group were restored just using the D.T.Light- Post. Clinical results and X-ray were recorded after 18 months. Results There was no post crown fall off or loosed in the experimental group, the successful rate was 100.0%. There were one post crown fall off in the control group, and four post crown loosed, the successful rate was 88.1%. The experimental group and control group have the significant differences. Conclusion The D.T.Light-Post and auxiliary pole have good effect on the anterior teeth of residual root.

    Objective To evaluate effect of Chlorhexidine in preventing dry socket after impacted tooth extraction based on best evidence, and guide its clinical use. Methods Using Chlorhexidine and dry socket as keywords to search evidence-base medicine reviews from Cochrane library and American College of Physicians Journal Club to find the best relevant clinical evidence and evaluate quality of the evidence. Results According to the evidence, Chlorhexidine of mass fraction 0.12% can effectively reduce the incidence of dry socket, but research on the effect of various usage or drug formulations for Chlorhexidine is insufficient, further study is needed. Conclusion Chlorhexidine of mass fraction 0.12% can effectively reduce the incidence of dry socket in impacted tooth extraction.

Silk protein material has been used as an ideal scaffold material for cell adhesion and growth in bone tissue engineering. This review summarizes investigations on the structure, biological characteristics of silk protein and its application in bone tissue engineering.

    Periodontitis is a chronic infection caused by bacteria in dental plaque which is characterized by destruction of periodontal tissues caused by direct aggression of bacteria and host response. When periodontal tissues were stimulated by bacteria, a series of inflammatory signal pathways were activated and the expression of inflammatory mediators increased. Therefore, development of periodontitis may be regulated via the mediation of signaling pathways and inflammatory mediators. There were substantial researches concerning immunomodulatory treatment, and this article aimed to review new progress of immunomodulatory treatment applied in periodontitis.

    Transforming growth factor(TGF)-β superfamily signaling pathways not only regulate numerous physiological processes such as embryonic development, angiogenesis and wound healing, but also play an important role in the regulation of cell proliferation, differentiation, maturation and programmed cell death. In addition, signaling by TGF-β superfamily also regulates chondrogenesis and cartilage development. Smad protein mediate signal transduction induced by the TGF-β superfamily members by transmitting the signals from the cell surface to the nucleus. In this review, we focus on the classification of Smad, the mechanism of the transduction of TGF-β superfamily signals by Smad, the mechanism of the Smad signaling regulates the chondrogenesis and the negative regulation of Smad signaling.

    Gap junction is a kind of membrane channel. It is widely spreaded in the bone which is updated and responds to the mechanical strain. Osteocytes transmit the mechanical signals to osteoblast and osteoclast which realize bone remolded, after it sensing the signals. Gap junction plays a very important role in the process of converting mechanical signals into biological effects. This article reviewed the discovery and the testing methods of gap junction, structures and functions of it, the function of mechanism of how the mechanical signals turn into biological effects.

    Due to the unremitting study of the dentine adhesive, the phase separation phenomenon during bonding process becomes a hot issue in recent years. It is known that Dehydration on dentin due to establishment of hydrogen bond in peptide of collagen fiber. It doesn′t benefit for the formation of hybrid layer and resin tags. On the contrary, water-rich condition in dentin may cause the phase separation between hydrophilic and hydrophobic monomers. It will reduce the bonding strength and the bonding durability between the dentin and composite resin ultimately. Thus, reducing phase separation of adhesives in the presence of water and keeping homogenous penetration of adhesives into wet demineralized dentin are very critical factors during the dentin bonding process. Therefore, the research of phase separation phenomenon in dentine adhesive will be report in this review.

    Signal transducer and activator of transcription 3(STAT3) is constitutively activated in many human cancers which has been recognized as one of the common pathways. Numerous cytokines, growth factors, and oncogenic proteins activate STAT3. STAT3 signaling affects the expression and function of a variety of genes that are critical to cell survival, cell proliferation, invasion, angiogenesis. So, regulating the STAT3 signaling pathway constitutes a potential preventive and therapeutic target for cancer metastasis.

    Altered loading is an important etiological factor for the temporomandibular disorders. Through changing the loading, chondrocyte can secrete various cytokines and enzymes to control the synthesis of cartilage matrix and decomposition. Recent years,researches have found that the change of the subchondral bone plays an important role in the progression of osteoarthritis. Subchondral bone sclerosis is not only the result of osteoarthritis(OA) but associated with the development of it. For the therapy of OA, we should concerned about the cartilage and the prevention for the degeneration of the subchondral bone. In this article, the anatomic structure,physiological function, methods of altered loading and the effect on the subchondral bone and cartilage have been reviewed.

    Bone tissue engineering is the technology culturing and multiplying bone marrow stromal cells, then inoculating cells into the scaffolds and implanting the cells -scaffolds compounds into the human body. Today, more and more artificial biomaterials have been utilized in the research of bone tissue engineering, and their effects that account for cells proliferation and osteogenesis need to be evaluated. This paper summarizes some detection methods frequently used before and recently, analyzes their respective advantages, disadvantages, and application prospects and makes a review about the evaluation methods of the bone tissue engi neering.

    Tissue engineering is the construction of new tissue and organs for reparative purposes. In order to implant tissues of greater size to survive, the tissue has to be vascularized, and implementing the vascularization as early as possible is pivotal to the success of the engineered tissue. Adipose tissue engineers use two main methods to create a vascularised structure: Stimulation of the endogenous angiogenic responses by the vascular growth factors, and formation of structures that mimic the vascular tree of the organ or tissue to be replaced. We will illustrate the growth factors of vascularization utilized for obtaining optimal results in the field, and the fabricating methods of the scaffold using these vascular growth factors in vivo and in vitro.

    Color measurement instruments are able to quantify the surface color of everything. This method is objective and accurate. The two specialities of the instruments are the basis of the using in prosthodontics. Recent years, with the development of maxillofacial prosthetics, the simulation of restoration has become a goal to pursue in current prosthodontics. Patients are no longer limited to meet the application of functional restoration, but pay more attention to the aesthetic requirements. In this paper, we write a review about the color measurement instruments and their applications in stomatology.

    With the improvement of living standards and esthetic requirements, patients have more demands on esthetic of dental restorations. Compared with other cosmetic techniques, porcelain laminate veneers have many advantages, such as least tooth reduction, good biocompatility, and color stability. The restorations have gained popularities for all of the conservative restorations. While, the final aesthetic effect may be influenced by kinds of factors. The paper reviews the history of clinical applications, the choice of indications and contraindications and types of materials, the tooth preparation methods, the adhensive system, in order to have the theoretical bases gained the popularity of clinical applications.

    Osteoarthrosis is a common degenerative articulation disease, which is characterized by cartilage injury. Conventional treatment of this disease is not satisfactory. Gene therapy becomes a new technique for the treatment of joint diseases in recent years. Temporomandibular joint is one of the most active synovial joints. It is in ongoing remolding after birth, which will meet to the demand of masticatory function at different age stages. The structure and function of this joint are significant to keep the balance of stomatognathic system. Although temporomandibular joint is small in size, it bears relatively heavy masticatory force. So it is more vulnerable to osteoarthrosis than other larger joints. This article is a brief review about the progress of target cells, candidate gene and vectors of gene therapy on temporomandibular joint osteoarthrosis.

    The implantological rehabilitation of the esthetic zone is one of the most demanding and complex treatments due to the necessity to obtain an optimum esthetic result. At the level of the soft tissues, it involves to obtain the complete formation of the papilla and to create a harmonic contour of the gingival margin. And this result should be predictable and stable, and it depends on the interaction of multiple variables that include biological factors, relating to the implant and prosthodontics. This review summarized the investigative situation and research progression of factors that influence the position of the periimplant soft tissues.

    The psychology impact on hospitalized children with cleft lip and palate about medical staff′s clothing and behaviors

    The paper aims for how the medical staff′ s clothing and behaviors affects the psychology of the hospitalized children with cleft lip and palate. First, we analyze the characteristics of the hospitalized children, and alter the medical staff′s clothing and behaviors. At the same time, we also decorate the circumstance of our hospital. Then we analyze the hospitalized children′s psychological status. The results show that altering the medical staff′s clothing, behaviors and the circumstance of the hospital can release the psychological pressures of the hospitalized children with cleft lip and palate which can cooperate well during the treatment.
