    Review and prospect of digital all-ceramic restorations

    In recent twenty years, the physical and mechanical properties and the operation process of dental ceramic materials have gotten a huge development. All-ceramic restorations have good esthetics and biocompatibility, so they are replacing porcelain fused metal restoration gradually. Technology of digital all-ceramic restorations use computer aided design and computer aided manufacture(CAD/CAM) to manufacture all-ceramic restorations. At present, a lot of all-ceramic restorations are completed by digital technology which is a hot spot in recent researches and clinical applications. This article will summary and prospect of digital all-ceramic restorations by necessity of digital all-ceramic restorations, CAD/CAM, design and manufacture of restoration, material selection, all-ceramic bonding techniques.

    The efficacy of Vitapex paste filler for sinus-type periapical periodontitis patients with elderly diabetes

    Objective To observe the reaction after 1 week and clinical efficacy after 2 years by using Vitapex paste filler treated elderly diabetes patients with sinus-type periapical periodontitis. Methods 225 teeth selected from 144 patients with elderly diabetes and sinus-type periapical periodontitis. When the preparation and sterilized root canal were finished, the teeth were divided into two groups: Group A of 72 patients with 116 teeth treated with Vitapex paste filler and gutta, group B of 72 patients with 109 teeth treated with Cortisomol paste filler and gutta. The reaction after 1 week and clinical efficacy after 2 years were observed. Results There weren’t significantly difference between two groups after 1 week(P>0.05); the effective difference between the two groups was significant after 2 years, the clinical efficacy of group A was obviously better than group B(P<0.05). Conclusion The short-term emergency response is light and long-term efficacy is good by using Vitapex paste to treat the sinus-type periapical periodontitis with elderly diabetes.

    Objective To evaluate the effect of oxidation on the hydrophilic property of the medical nickel titanium( NiTi) alloy. Methods The test samples were divided into two groups as a oxidized group and a non-oxidized group. NiTi alloy with nearly equiatomic ratio of nickel and titanium elements was selective oxidized in boiled 30% hydrogen peroxide solution solution. Then, the oxidized alloy was ultrasonically rinsed with deionized water and dried at 37 ℃. Static contact angle was measured using contact angle measuring instrument by stop drop method. The samples were placed in the sample room. Deionized water and α -bromonaphthalene were dropped on the samples surface. Then the measured pictures were acquired and the contact was evaluated by the accompanying software. Results The surface contact and interfacial tension of the oxidized NiTi alloy was lower than those of the non-oxidized while the surface tension was higher. The blood compatibility of the NiTi alloy will be improved by surface oxidation concerning wetting ability, interfacial tension and surface tension. Conclusion Surface thermodynamics properties including wetting ability, surface tension and interfacial tension are improved by oxidation treatment which leads to the improvement of blood compatibility of the NiTi alloy.

    Objective To describe the anatomy of fibular flap to provide anatomical basis for reconstruction of maxillomandibular defect. Methods Six adult lower leg specimens fixed by 10% formalin were dissected layer by layer, and then observing, measuring and taking photographs. Results The original external diameters of peroneal artery were (2.93±0.47)mm, the perpendicular lengths from fibular head to original peroneal artery and to fibular perforator vessel were (41.75±3.17)mm and (166.67±17.23)mm, respectively. All peroneal arterys originate from the posterior tibial artery. Conclusion Vascularised fibular flap adapts to maxillomandibular reconstruct due to itsvariable course of peroneal artery and wide original external diameters relatively, as well as invariable perforator vessel relatively.

    Objective The aim of this retrospective study was to evaluate the clinical efficacy of treatment protocol with extraction of maxillary first premolar and the mandibular second molar in the correction of bony class Ⅱ open bite. Methods 12 patients with bony class Ⅱ open bite cases, 4 males and 8 females, mean age is 15.6 years old, they were treated with extraction of maxillary first premolar and the mandibular second molar and the straight wire appliance. All the lateral cephalometric photos before and after treatment were analyzed. Results Satisfactory results were achieved in 12 patients after treatment, significant improvement of facial profile, normal bite, normal jet and counterclockwise rotation of the occlusal plane were achieved, mandibular plane angle became smaller. Conclusion The clinical efficacy of treatment protocol with extraction of maxillary first premolar and the mandibular second molar in the correction of bony class Ⅱ open bite was effective and stability.

    Objective To explore the method of orthodontic treatment of inverse impacted maxillary central incisor to improve clinical treatment effect and retain more inverse impacted incisor. Methods 8 patients with 9 inverse impacted maxillary central incisors were treated with methods of opening space, surgical fenestration and traction, fixed artificial fulcrum to adjust direction aligned, reverse root control and so on. Results Inverse impacted maxillary central incisors have normal position, stability teeth root and proper function after 15 to 30 months of orthodontic treatment. Another incisor without clear displacement was extracted after 15 months of orthodontic traction, because its root was short and bend 180°. Conclusion Inverse impacted maxillary central incisor with normal root or slightly short root, or bent root less than 100° can be retained and keep normal function completely through a series of orthodontic treatment. Fixed adjustable artificial fulcrum can assist in treating inverse impacted maxillary central incisor. It can effectively shorten the course of treatment and improve efficacy.

    Objective To assess the feasibility in production of individualized lingual bracket system using computer aided design/computer aided manufacturing(CAD/CAM) technique. Methods The cast model was scanned by optical 3D scanner and 3-dimension digital model was reconstructed with appropriately designed software. The individualized lingual brackets could be designed with appropriate design software(CAD program). Using high-end rapid prototyping machine and casting technique, the virtual bracket series were converted into metal ones. According to the archwire geometry designed by CAD technique, the individualized lingual archwires were manufactured. Results The reconstructed 3-dimension digital model could be suitable to design the virtual individualized lingual bracket and archwire geometry. The virtual individualized lingual bracket and archwire geometry are in accordance with a real-life metal brackets and archwires on the cast model. Conclusion The use of CAD/CAM technology can produce individualized bracket system in lingual orthodontics.

    Objective To evaluate the value of triptolide as an antitumor drug by observing its inhibitive effect on human oral squamous cell carcinoma(cell line Tca8113), and to compare it with conventional chemotherapeutics cisplatin. Methods Methyl thiazolyl tetrazolium(MTT) assay was used to examine the inhibitive effect of triptolide on the proliferation of Tca8113 in vitro. The morphology of the cells treated with triptolide was observed by inverted phase contrast microscope. Results Triptolide had significant effect on Tca8113 in a dose-dependent and time-dependent manner(P<0.05); Lower concentration of triptolide could exert considerable inhibitive effect, the inhibitive rate of 0.001 mg·L-1 and 0.01 mg·L-1 triptolide was 60% and 90% at 7 days, respectively statistically significantly better than those of cisplatin; while the concentration more than 0.1 mg·L-1, cisplatin was superior to triptolide in inhibiting the cells proliferation at 1, 3, 5 days(P <0.05); the 50% inhibitory concentration of triptolide was less than those of cisplatin at every time point. Conclusion Triptolide has significant suppressive effect on human oral squamous cell carcinoma cell line Tca8113 in dose- and time-dependent manner(P<0.05), and it works better than cisplatin at low concentrations with longer acting time.

    Objective Basing on the three-dimensional reconstruction of jaw bone data, a template was designed for the guide of implantation of miniscrew. It’s postoperative accuracy and clinical results were also evaluated. Methods The CT scan was performed on 30 patients who needed imbed implant anchorage. Basing on the data, three-dimensional model was reconstructed and the location of implant was determined. The location plates basing on the labial and palatal surface data and guide tube that can be assembled were designed. All data we obtained were output to make to resin template using rapid prototype technology. One side of each patient’s maxillary were randomly selected to imbed miniscrew by using three-dimensional model. The other side located miniscrew by X ray film. The effect of template were evaluated during 5-7 months treatment. Results The template we designed can do the stable guide. The miniscrew can reach the exact place as anticipated designed. After 5-7 months of clinical observation, 30 miniscrew position and orientation of experimental group consistent with the preoperative design, and no contact with adjacent teeth. A case of failure according to the interference by soft tissues, and the success rate was 96.7%. There were 23 miniscrew stability and 7 loosening in the control group. The success rate was 76.7%. Conclusion The miniscrew can be implanted accurately to the optimal location with three-dimensional guide and decrease the failure rate of implant anchorage.

    Objective To investigate the microstructure and properties of titania(TiO2) and zirconia(ZrO2) coated on titanium and to test its vitro cytotoxicity. Methods The titanium specimens were divided into test group and control group, and test group was coated with a TiO2/ZrO2 complex film on the surface by plasma spraying technology. The microstructure of the coating was analyzed by energy-dispersive spectrometry and scanning electron microscope. The metal-porcelain bond samples were made from both groups and Titankeramik porcelain which were used for metal-porcelain bond strength measurement. The extraction liquid of the coating was obtained and cell relative growth rate(RGR) was calculated through methyl thiazolyl tetrazolium assay by which the cytotoxicity was graded. Results The TiO2/ZrO2 coating were well bonded with titanium and no obvious fracture was observed. The metal-porcelain bonding strength of test group were higher than control group significantly(P<0.05). The toxicity gradation of all groups was 0-1, RGR of all groups were almost the same and increased with culture time. Conclusion The results indicate that the TiO2/ZrO2 coating are well bonded with titanium which can improve the metal-porcelain bond strength of titanium and is compatible with the tested cells.

    Objective To study mitochondrial transcription factor A(Tfam), 8-oxoguanine DNA glycosylase(Ogg)-1 and DNA polymerase gamma(Polg), which is relative to mitochondrial DNA repair system, regulated by γ-irradiation in oral squamous cell carcinoma(OSCC). Methods OSCC-2 and 5 cells were cultured in vitro and treated with γ-irradiation. Cell survival rate was calculated with methyl thiazolyl tetrazolium assay. Using Western blot, PCR and RT-PCR detected the protein and mRNA level of Tfam, Ogg-1 and Polg. Results After OSCC-2 and 5 cells were irradiated at a dose of 30 Gy, the cell proliferation were (35.33±6.03)% and (75.67±4.16)% respectively. The level of tfam mRNA were increased, but higher tfam mRNA levels were observed in OSCC-5 cells at 6 h after irradiation. Besides, the expression of Ogg-1 and Polg was also stronger in the irradiated OSCC-5 cells. In addition, each small interfering RNA(siRNA) down-regulated the corresponding protein expression. Transfection of the siRNA increased apoptosis of the irradiated cells. Conclusion In vitro, γ-irradiation can inhibit the proliferation and up-regulate the expression of Tfam, Ogg-1 and Polg of OSCC-2 and 5 cells.

    The differentiated epithelial cells could be transformed into mesenchymal cells through a cellular program named epithelial—mesenchymal transformation(EMT), which plays an important role in development, carcinoma invasion and also chemoresistance. Since chemoresistance always cause the failure of chemotherapy, to overcome chemoresistance is key to maintain the effectiveness of chemotherapy. This article reviews EMT and its biological significance, its association with chemoresistace, especially in head and neck cancer.

    Traditional dentin water wet-bonding technique has been advocated by many scientists, but the surface wetness is difficult to control, the excess water will induce suboptimal polymerization of those adhesives, phase separation and nanolekage, which will influence the longevity of resin-dentin bonds. Recent studies have put forward a new concept which is dentin ethanol wet-bonding, this technique can increase in bond strength and durability. This review is focus on the principle of ethanol wet-bonding, its surface treatment methods and the research progress on it.

    Receptor activator of nuclear factor -κB ligand(RANKL)/receptor activator of nuclear factor –κB (RANK)/osteoprotegerin(OPG) system is a key pathway in osteoclast differentiation, maturation and bone resorption. This system also plays a central regulation role in bone growing and remodeling as well as in pathogenesis of periodontitis. OPG is an important mediator for the balance of bone formation and resorption for that it can inhibit differentiation and bone resorption activation of osteoclast, and induce osteoclast apoptosis.

    Adipose-derived stem cells(ADSCs) are more abundant and easily accessible in harvest with less donor site lesion. Studies have observed the osteogenic differentiation potency of ADSCs induced by various means. Basing on the reports, the author reviews the biological characteristics and osteogenic induction methods of ADSCs, and the state of art of ADSCs bone tissue-engeering. Other potential influential factors such as gender, age and harvest location are also discussed. To date, ADSCs osteogenic differentiation has been identified in vivo and in vitro, however, further researches on amelioration of efficiency of ADSCs osteogenic differentiation and mechanism of those downstream adjusting genes involved in ADSCs bone tissue-engeering are still necessary.

    Silicone rubber is widely used in oral maxillofacial prosthesis and denture soft lining, because of its good physical, chemical, and biological properties. The low toxicity and surface porosity of silicone rubber provide a good condition for the proliferation of bacteria in oral environment. The adherence of bacteria, especially Candida albicans, can accelerate the aging and coloration of the material, affecting the esthetics and lowing the useful life of material, even cause denture stomatitis. So the research of antimicrobial effect of silicone rubber is one of the focuses in recent years. This paper summarized the current progress on the antimicrobial effect of denture soft liner and maxillofacial prosthesis silicone rubber.

    Activator appliance is a functional appliance used in orthodontic treatment early, which was designed in 1908 by Andresen. It can inhibit maxillary forward and stimulate mandibular growth to regulate the sagittal jaw relationship. Currently, there are many differences on the principles of Activator appliance, one of which is whether it stimulates the growth of mandibular condyle to increase the height of ramus and mandibular length. In recent years, scholars carried out many clinical studies and animal experiments in the field. This paper reviews the effect of Activator appliance on temporomandibular joint, and discusses the relationship between Activator ap - pliance treatment and temporomandibular disorders.

    Hypophosphatasia(HP) is a rare inherited disorder characterized by defective bone and teeth mineralization, and deficiency of serum and bone alkaline phosphatase(ALP) activity. The prevalence of severe forms of the disease has been estimated at 1/100 000. The symptoms are highly variable in their clinical expression, which ranges from stillbirth without mineralized bone to early loss of teeth without bone symptoms. The diagnosis is based on DNA sequencing of the ALP gene. In recent years, the research direction included tissue nonspecific alkaline phosphatase and its function of bone mineralization, the function of gene mutations in HP, the effects of HP’s treatment.

    Maxillary protraction and distraction osteogenesis are commonly used in cleft lip and palate patients with maxillary hypoplasia during prepuberty. The effect of the former one is dealing with the timing and the appliance selected while the latter one is associated with the types of distraction osteogenesis including face mask distraction, rigid external distraction and internal distraction. The long term stability of both treatment techniques is still under controversy. This paper reviews the effect and long term stability of the maxillary protraction and distraction osteogenesis.

    Platelet -rich fibrin(PRF) is a new generation of platelet concentrate developed by Choukroun in 2000. Its processing is simple and without any additives, reducing the risk of infectious diseases’transmission. PRF fibrin network presents biochemical architecture of connected trimolecular or equilateral junctions. Thus, the flexible and elastic fibrin network is able to support cytokines enmeshment and cellular migration. It is rich in platelets and cytokines that can promote tissue healing and regeneration. It can also regulate inflammatory reactions. We present a review on PRF’s biological characteristics, regulation mechanism and clinical applications.

    In the research of pathogenesis of diabetes-related periodontitis, oxidative stress could be activated by long-term chronic hyperglycemia of diabetics via multiple processes, which contributes to accumulation of reactive oxygen species(ROS) in periodontal tissues. While ROS alters signal transduction path, leading to abnormality of inflammatory factor’s gene and protein expression, which amplify host inflammatory response, ultimately perioodontitis occur. The purpose of this review is to provide an overview of the research of concerning pathogenesis of diabetes-related periodontitis, in regard of different path of oxidative stress activation, mechanism of oxidative stress and therapeutic prospect of antioxidums in treatment of diabetes-related periodontitis.

    Sialolithiasis is a common disease of salivary glands, mainly afflicting the major salivary glands. Characterized by the obstruction of the salivary secretion by a calculus, its main symptoms are recurrent swelling and pain, and may be aggravated during eating. Owing to the rapid developments in endoscopic techniques, it is now possible to perform diagnostic and interventional procedures directly within the salivary ductal system with less invasiveness. The etiology of these calculi and the exact mechanism of formation, however, are unclear. Sialoliths are composed of a central mineralized core and laminar layers of organic and inorganic substances. Irritant factors trigger a response and cause spasmodic contraction of the canal and stasis of saliva and then lead to the development of calculus. During the development of sialoliths, multi-factorial events can be involved, including secretory disturbances(microlith formation, ductal obstruction and anatomical abnormity) and influence of saliva(high calcium affinity, decreased concentration of crystallisation inhibitors, influence of pH, diversification of bacteria and components of protease).

    Self-etching adhesive system has good prospects in the adhesive of dentin and enamel with its advantages of easily operating, good sealing characteristic of the restoration margin and low technique sensitivity. In the clinic, dental defect resulting from caries is the main component of restoration. Dentists supposed that cariesinfected dentin should be removed, but the dentin demineralization without bacterial invasion(caries-affected dentin, CAD) should be reserved. CAD is the major proportion in bonding. The mineralization of CAD is lower than the normal dentin, which may lead to a different resin bond effect between CAD and normal dentin. This paper reviewed the research progress on the role of self-etching adhesive system in the bonding of CAD, contributing to popularize its internal clinical application.

    We recently showed that adiponectin, an adipocyte derived cytokine, may function as a type Ⅱ diabetes protective factor. It had an effect of lowering blood glucose, increasing insulin sensitivity, anti-inflammatory, anti-atherosclerosis and regulating the bone metabolism. Periodontitis is considered as the sixth complication of diabetes. Some researches indicated that there lies a close relationship between periodontitis and diabetes, which often influence each other. The interaction of adiponectin and periodontitis is not clear. In this review, in order to acquire a new therapy of periodontitis in the future, the influence of adiponectin on type Ⅱ diabetes patients with periodontitis will be discussed.

    Black triangle is common phenomenon in our dental clinical treatment, the etiology, diagnosis and treatment of which involving periodontics, prosthodontics and orthodontics. During the dental treatment, we should strengthen the understanding, pay attention to multidisciplinary cooperation, in order to prevent the happening of black triangle effectively. As the increasing of esthetic consciousness, patients put a high demanding on orthodontic treatment. The occurrence of black triangle after orthodontic therapy would not only be unesthetic, but also adversely affect the health of periodontium, occlusal relationship and stability of teeth arrangement. In this article, the prevalence of anterior black triangle after orthodontic treatment, risk factors, diagnosis, treatment and prevention were reviewed, in order to provide a reference for the orthodontic clinical.

    Jaw relationship recorded for patients who need complete denture again includes the vertical dimension, the horizontal relation, and the hinge axis relation. For edentulous patients with denture repair again, who use the old dentures for many years, most of their jaw relationships is characterized as the inconformity between intercuspal position and muscular contact position. If not to adjust the jaw improper position, it can eventually induce or aggravate temporomandibular joint disorder syndrome. Therefore, to determine the appropriate jaw relationship is the way to prevent and treat temporomandibular joint disorder syndrome, and the key to do the restoration successfully. In this paper, the research progress on jaw relationship, which includes the vertical dimension, the horizontal relation, and the hinge axis relation were reviewed.
