    The history, present situation and development of the maxillary sinus lifting

    Implant placement in the posterior maxilla is often limited by the lack of vertical bone height. Maxillary sinus floor elevation is the effective way to resolve the inadequate bone height in the posterior region of the maxilla in order to promote the success of implants. In the 1960s, Philip Boyne recommended the lateral window(LW) approach: Caldwell- Luc technique, elevation of Schneiderian membrane, in order to increase the vertical bone distance. Robert Summers in 1994 introduced the osteotome technique(the transalveolar approach), which is less invasive for sinus floor elevation. Now, The lateral approach and the transalveolar approach developed into two main methods of Sinus Lift technology. This article reviews the historic, present situation and development of sinus lift techniques and discusses the improvement of the operative indication, surgical technique and the surgical instruments.

    Matrix metalloproteinase 2 expression in implant-bone interface remodeling influenced by submerged and nonsubmerged implant denture

    Objective This study was designed to investigate the matrix metalloproteinase 2(MMP2) expression in implant-bone interface remodeling after loading the submerged and nonsubmerged implant dentures. Methods Eight adult beagle dogs were used to construct the animal model. Submerged and nonsubmerged implants were implanted into the bilateral mandible. Fixed metal full crown was used to load the submerged and nonsubmerged implant denture. The beagle dogs were sacrificed after loading. Immunohistochemistry was used to observe the dynamic MMP2 expression in the implant-bone interface. Results MMP2 expression in the implant-bone interface of the experimental group was significantly enhanced compared with that of the control group after loading of implant denture. MMP2 expression in the interface was observed 2 weeks after implant denture loading, but no statistically significant difference was found between the experimental and control groups. MMP2 expression in the experimental group was significantly higher than that in the control group at 4 and 8 weeks after loading, and the peak was reached at 4 weeks. Then, the MMP2 expression in the experimental group declined to the level of the control group after 12 weeks. No obvious difference was observed in the MMP2 expression in the implant-bone interface between the submerged and nonsubmerged implant denture. Conclusion The mechanical stress that was transferred from the implant to the bone tissue after loading of the submerged and nonsubmerged implant denture can stimulate MMP2 expression in the implant-bone interface, promoting interface remodeling. Furthermore, no distinct difference was observed in the implant-bone interface remodeling after the loading of submerged and nonsubmerged implant denture.

    Objective This study aims to establish the obstructive sleep apnea syndrome(OSAS) animal model in rats with periodontitis and to explore the correlation between OSAS and periodontitis. Methods Twenty SD rats were randomly divided into the control group(group A) and intermitted hypoxia exposure group(group B). The oxygen concentration in the cabin ranged from 10%±0.5% to 21%±0.5% through the circular input of oxygen and nitrogen for 8 h daily. Each SD rat received wire ligatures around the cervix of the left second maxillary molar, whereas the contralateral tooth was untreated as control. The rats were sacrificed after right weeks. X-ray and histological examination were used to evaluate the periodontal breakdown. Results The periodontal depth, sulcular bleeding index, and alveolar bone loss of the ligatured teeth significantly increased, especially in group B. Conclusion Intermittent hypoxia can aggravate periodontal inflammation and may also be a risk factor for periodontics.

    Clinical application of submental island flap to repair the defect during the same period after oral cancer surgery

    Objective This study aimed to observe the clinical effects of submental island flap(SIF) for repairing oral defects after oral cancer surgery. Methods A retrospective analysis of 60 cases of postoperative oral cancer defects in patients with clinical data was performed. The experimental group and the control group were subjected to treatment(30 cases in each group). SIF was used to repair postoperative defects in the experimental group. By contrast, the defects of the control group were treated by using the radial forearm flap after surgery. A significant difference occurred between the two groups. Results The clinical efficacy of the 25 cases in the experimental group was excellent, and 4 cases were good. Approximately 96.7% of the cases in the experimental group achieved excellent and good ratings. In the control group, 20 cases were excellent, and6 cases were good. Approximately 86.7% of the cases in the control group achieved excellent and good ratings. Thus, the two groups had no significant difference(P>0.05). The mean operation time in the experimental group was (45.72±7.64) min. The mean operation time in the control group was (60.14±10.68) min. Thus, a statistically significant difference(P<0.05) was observed. The average bleeding volume was (23.33±4.13) mL, whereas the mean bleeding volume obtained from the control group was (27.79±5.38) mL. Therefore, a statistically significant difference(P<0.05) was exhibited between the two groups. The findings indicate that the two groups were not associated with the clinical complications of cervical incision. Conclusion The application of SIF for treatment of oral cancer postoperative defects is advantageous because it is a simple and minimally invasive procedure. In addition, using SIF provides short operation time, allows less amount of intraoperative bleeding, and conceals the wound caused by flap insertion.

    Clinical application of an efficient root torque auxiliary arch of individual tooth

    Objective To compare the postoperative clinical efficacy between this new root torque auxiliary arch and the conventional method which added torque on the wire where it needed and analyze the reasons. Methods Thirty patients who needed to control the movement of the root of individual tooth were randomly divided into 2 groups. Group A took conventional method which added torque on the wire where it needed, while Group B took this new root torque auxiliary arch. Then compared the rates of efficiency and the time of root movement therapy. Results There was no significant difference between these two groups in efficient rates(P>0.05). But the time of root movement therapy in Group A was (25.50±4.19) weeks, while (12.12±4.67) weeks in Group B. There showed significant differences(P<0.01). Conclusion This new root torque auxiliary arch was not only an efficient approach to the movement of the root of individual tooth, but also shortened the treatment time.

    Objective This study is aimed to get the gene expression signatures of calmodulin by mining data from Oncomine database as well as proving them with our samples. Methods Differential calmudulin mRNA expression levels in the head and neck squamous cell carcinoma(HNSCC) were got from Oncomine database, and the expression of calmodulin on mRNA and protein level of the clinical samples and oral carcinoma cell lines was analyzed by real-time polymerase chain reaction and Western Blotting, respectively. Results The results indicated that in most of databases of HNSCC samples compared to adjacent non-tumor(or normal) tissue presented a significant increase in calmodulin mRNA level, but only a few not. The results from our experiments show that the expression of calmodulin on mRNA and protein level of normal tissue is more higher than that of tumor tissue. Conclusion We can get the information about the expression of calmodulin mRNA level on HNSCC quickly and exactly by combining data-mining from Oncomine database with our verified experiment. This can make a ground for the developmental research about calmodulin.

    The success rate evaluation of 3 orthodontic adhesives as occlusive pad

    Objective Compare the success rate of 3 different types of orthodontic adhesives as occlusive pad. Methods One hundred and eighty-nine patients, with a total of 480 teeth were evaluated, 3 different types of orthodontic adhesives were applied as occlusive pad. The sample were randomly divided into 3 groups, 160 teeth per group. Group A used glass ionomer cements, group B applied GC Fuji LC, and group C used Resilience. We followed up for 6 months, and compared the detachment and success rate of orthodontic adhesives. Results The success rates of Resilience, GC Fuji LC and glass ionomer were 96%, 87%, 75%, respectively, presenting significant statistical difference(P<0.05). Conclusion Significant differences existed among 3 orthodontic adhesives in terms of detachment and success rate.

    Objective To study the number and incidence of the lateral root-canals and accessory root-canals in localized periodontitis teeth. Explore the role of endodontic lesions in the process of localized periodontitis. Methods Thirty teeth with localized periodontitis(experiment group) and 30 teeth with generalized periodontitis(control group) were collected according to a coincident standard. Before the teeth were extracted, the alveolar bone absorption was analyzed by imageological examination before the experiment. The transparent specimen was made in vitro teeth. The lateral root-canals and accessory root-canals were counted under a dissecting microscope. Results The statistical analysis revealed that the incidence of angular alveolar resorption in the teeth with localized periodontitis(76.7%) was greater than that in the teeth with generalized periodontitis(36.7%) obviously. The incidence of the lateral and accessory rootcanal was 80% and 52% in the teeth with localized periodontitis and generalized periodontitis respectively. The difference between the two groups was significant. Conclusion The lateral root-canals and accessory root-canals were more in localized periodontitis. The role of endodontic lesions and the excess anatomic pathway in the process of localized periodontitis was very important.

    The comparative survey and analysis among Chengdu and Xi’an residents on cognition of implant dentures

    Objective Survey the cognition of implant dentures of Chengdu and Xi’an citizens, and comparatively analyze the differences and influencing factors. Methods A questionnaire was designed for this study. Its main content included basic demographics of respondents(gender, age, cultural level, and income level), and cognition level of implant dentures(concept and main characteristics of implant dentures). The sampling survey was carried out randomly among Chengdu and Xi’an citizens in November, 2013 using the designed questionnaire. The results were then statistically analyzed. Results There were 905 valid questionnaires in Chengdu and 945 ones in Xi’an. The average score about the cognition level of implant dentures of Chengdu residents was 7.12±3.36 and it was 6.98±2.96 of Xi’an residents. The influencing factors of cognition of implant dentures are the cultural level, income level and age. Conclusion The citizens’ cognition of implant dentures is not high in both Chengdu and Xi’an. Chengdu citizens’ cognition is slightly higher than the ones in Xi’an. The effective measures to increase the cognition level of implant dentures may include strengthening doctor-patient communication, enhancing public health education, improving medical technology, and reducing the cost of implant dentures.

    Objective The aim of this study was to observe the clinical effect of an in-office desensitizing polishing paste containing 8% arginine and calcium carbonate on the treatment of root-dentin hypersensitivity after subgingival scaling with ultrasonic instrument. Methods This was a randomized double-blind controlled, clinical study. The 82 subjects who presented at least two hypersensitive teeth either front teeth or premolars with an air blast hypersensitivity scores of 2 or 3 according to schiff cold air senditivity scale after subgingival scaling with ultrasonic instrument in one week were randomly stratified into two groups. 40 subjects in test group were treated with an in-office desensitizing polising paste containing 8% arginine and calcium carbonate. 42 subjects in control group were treated with the proxyt polishing paste. The effect of desensitization was evaluated immediately after treatment. Results Eighty-two subjects participated and completed in this treatment. The average air blast hypersensitivity score of test paste was 1.34±0.48. The average air blast hypersensitivity score of control paste was 2.24±0.55. There was significant difference between control and test group(P<0.05). Conclusion The in-office desensitizing polishing paste containing 8% arginine and calcium carbonate showed good therapeutic efficacy for patients with root-dentin hypersensitivity after subgingival scaling with ultrasonic instruments.

    Clinical study and evaluation of temporary dentures for posterior tee

    Objective To study and evaluate the clinical effects of immediate soft-lining on temporary dentures for posterior teeth. Methods Selected 58 patients with temporary dentures treated in our hospital between July 2009 and May 2013, and they are randomly divided into test group and control group. Wipe off the teeth only on the stone model in the test group, and soft-lining on the temporary dentures when patients wearing it. Modificate the stone model according to Felly principle, the patients wear the dentures directly after extracted the teeth in the control group. They were all followed up at the 1st week, the 5th week and the 9th week, the clinical effects of the two groups were compared. Results The clinical effects of test group(86.2%, 82.7%) were significantly better than those of control group(62.1%, 58.6%) at the 1st week and the 5th week(P<0.05). But there are no significant differences between the two groups at the 9th week. Conclusion The clinical effects of immediate soft-lining on temporary dentures for posterior teeth are very well, it can applied widely in clinic.

    Patients with a cleft lip and palate occasionally experience mental disorders in clinics. This study presents a case of an adult patient with a cleft lip who manifested acute psychiatric symptoms after operation. These symptoms include auditory hallucinations and delusions. The psychological diagnosis, counseling, and treatment of professional counselors helped to alleviate the mental disorders of the patient, who exhibited satisfactory treatment results.

    Autism is a psychological development disorder that starts from infancy. Autistic patients exhibit impaired social and communication abilities, limited interests, and stereotyped repetitive behavior. These patients have difficulties with self-care and cooperation during dental treatment, which are challenging for dentists. Familiarization with the oral characteristics of autistic patients will lead to better dental treatment results. This review investigates the oral diseases in autistic patients. These diseases include caries, factitial oral lesions, orofacial pain, and periodontal diseases. Intervention measures, such as oral hygiene instruction and dental treatment, were also examined.

    Nanomodifications of implant surfaces, which include nanoparticle compression, laser sintering, surface chemical process, and anodic oxidation, are widely adopted to enhance bone integration and reduce wound healing time. Many studies reveal that polymorphonuclear leukocytes and macrophages, as well as mononuclear and dendritic cells, play important roles in innate immunity. These cells are also involved in immune responses to implant surfaces. Furthermore, some cytokines can regulate immune responses to reduce inflammation, promote bone integration, and accelerate wound healing. This review presents methods of titanium implant nanomodification and the immune response of the body to nanomodified implants.

    Decreased bone density and osteoporosis are not favorable to orthodontic treatment. Estrogen can promote alveolar bone formation and inhibit bone resorption. Estrogen regulates bone remodeling through various cytokines under orthodontic force. First, estrogen can inhibit bone resorption and promote bone restoration by upregulating the expression of osteoprotegerin and downregulating the receptor activator of nuclear factor-κB ligand. Second, estrogen can promote osteoblasts and osteocytes to secrete bone morphogenetic proteins, which accelerate alveolar bone remodeling. In addition, estrogen can control alveolar bone resorptionby significantly inhibiting tumor necrosis factor-α, interleukin-1, and interleukin-6. Estrogen can also inhibit bone resorption by suppressing the expression of interferon-γ. Estrogen deficiency caninduce osteoclastogenesis by increasing the expression of macrophage colony-stimulating factor. Therefore, administration of optimum dosagesof estrogen is necessary in patients with osteoporosis for effective and safe orthodontic treatments.

    Mineral trioxide aggregate(MTA) and Portland cement(PC) contain calcium oxide, which reacts with water to form calcium hydroxide and producesa highly alkaline environment that inhibits bacterial growth. Compared with white PC(WPC) and MTA, gray Portland cement(GPC) has high concentrations of harmful heavy metals, namely arsenic, chromium, and lead. The solubility of root-canal filling affects itsability to close. The overall apical closure performances of MTA and PC are relatively similar. Good oral materials should have appropriate setting times. Addition of calcium chloride or calcium formate to MTA and PC shortens their setting time. The particle size of the material also affects its closure, setting time, compressive strength, and abrasion resistance. The average particle size of WPC is greater than that of MTA; however, the compressive strength of the former is lower than that of the latter. When mixed with gold powder, the compressive strength of WPC is similar to that of MTA. MTA and PC are non-toxic and genotoxic, and their toxicities to Chinese hamster ovary cells showno significant difference. Moreover, MTA and PC exhibit antibacterial activities toward Micrococcus luteus, Staphylococcus aureus, Escherichia coli, and other microorganisms. MTA and PC can promote hard tissue mineralization, which can leadto crown discolorations; PC results in lighter colors compared with those observed from MTA. Applications of MTA are limited by its long setting time, tooth discoloration, high-cost, and difficultyof removal. Thus, PC can be an economical alternative to MTA for dental restoration. The long-term effects of this material, however, are still uncertain and require longer assessments.

    The prominent components of halitosis are volatile sulfide compounds, especially hydrogen sulfide and methyl mercaptan, which can be produced by Helicobacter pylori(H.pylori). A possible relationship exists between H.pylori and halitosis from extraoral sources because bacterial eradication may resolve the symptom. However, the mechanism should be clarified. Gastroesophageal reflux disease is associated with halitosis, especially erosive cases. Almost 50% of dyspepsia patients develop halitosis, and the therapy of dyspepsia can lead to the disappearance of halitosis. However, this relationship remains unclear. Halitosis may be a significant concern in social situations because it is not only a health condition, but also has psychological effects that result in social and personal isolation. Given the undesirable condition of halitosis, the relationship between gastrointestinal diseases and halitosis should be investigated. Assistance should also be provided to the diagnosis and treatment of gastrointestinal diseases in relation to halitosis.

    Vascular endothelial cells are the core of the vessels. These cells express some specific markers, such as vascular endothelial growth factor receptor2, CD31, and the von Willebrand factor. These cells can promote the formation of tube structures in matrigel. Dental pulp stem cells has potential for self-renewal and differentiation into a variety of cell types, including osteoblast-like cells, chondrocytes, adipocytes, and neural-like cells in different medium with various content of inducer. Dental pulp stem cells also express endothelial cell markers and form vascular-like structure in vitro when cultured in low-serum culture medium added with vascular endothelial growth factor and fibroblast growth factor. In vivo studies suggest that these cells can promote angiogenesis and blood perfusion of regional ischemic tissue, which further confirms that dental pulp stem cells can also be differentiated into endothelial cells.

    Cell cycle and programmed cell death are closely related to the development of oral diseases. Cyclins, cyclindependent kinase(CDK), and cyclin-dependent kinase inhibitor are the major important moleculars for the regulation of cell cycle. Cyclin-dependent kinase inhibitor protein(CIP) and kinase inhibited protein(KIP) are constituted by P21, P27, and P57, which are involved in regulating cell cycle, cell death programming, and inhibiting the activity of CDK. Most of the tissue destruction of oral disease is caused by pathogens that disturb host cell proliferation and programmed cell death. The reduced or absent expression of CIP and KIP leads to junctional epithelium thickening, leukoplakia with epithelial dysplasia, and uncontrolled proliferation of tumor cells. Therefore, CIP and KIP plays an irreplaceable role in the regulation of gingival epithelial cell proliferation, the maintenance of periodontal tissues, and the expression level and activity of their work as an important target of canceration of oral leukoplakia and the progression and prognosis of tumors. This article focuses on the composition and function of CIP and KIP and the relationship between CIP-KIP and oral disease.

    MicroRNA(miRNA) plays a role in the progresses of early embryogenesis and proliferation, differentiation, survival, apoptosis, metabolism, and energy balance of cell. MiRNA21 promotes adipogenic differentiation of mesenchymal stem cell(MSC) and inhibits the proliferation of human adipose-derived stem cell by transforming the growth factor(TGF)β-Smad pathway. Bone morphogenetic protein(BMP) receptor(BMPR)2 is a target of miRNA21, which is necessary for BMP9-induced osteogenesis. This finding indicates that miRNA21 is related to the osteogenic differentiation of stem cell. During the differentiation of MSC, miRNA21 down regulates the expression of sprout(SPRY)1 and SPRY2 to mediate the activation and duration of extracellular signal-regulated kinase-mitogen-activated protein kinase pathway and improves the differentiation potential of MSC. The overexpression of miRNA21 can also enhance the survival of MSC under hypoxia or serum-free condition, but the down regulation of miRNA21 accelerates cell apoptosis. Thus, biological function and regulatory mechanism during the differentiation of miRNA21 stem cell can lay basis for the usage of stem cell in tissue engineering and the prevention and treatment of diseases.

    Transforming growth factor(TGF)β, a growth factor secreted by different cells, has been demonstrated to possess a variety of biological effects. Recent evidence has associated the TGFβ signaling pathway withthe regulation of angiogenesis and vascular stability. Extensive studies on the signaling pathway of TGFβ and its potential role in vascular regulation have been reported. In this review, the effects of the TGFβ signaling pathway on the regulation of vascular endothelial cells, smooth muscle cells, and vascular membrane, are discussed.

    The early stages of chronic kidney disease-mineral and bone disorders exhibit periosteal bone of jaw bones resorption, complete of partial disappearance ofperiodontal bony plates, thickening of cortical bone reduction, and the appearance of mental foramen, mandibular canal, and maxillary sinus anatomical structures. Periodontal bony plates also decreased or disappeared in 57.7% of hemodialysis patients with renal failure, maxillary osteoporosis, and local root bone sclerosis happened. Gingival crevicular fluid volume, osmotic pressure, plaque, and gingival index increasedin children with end-stage renal disease. Periodontal pathogens in chronic kidney disease patients were significantly higher than that of a healthy person. Thus, the risk of suffering from periodontal disease was higher. Plaque index, gingival index, and calculus index in hemodialysis patients significantly increased, and the severity of periodontal disease exacerbated with prolonged duration of dialysis. The calcium oxalate crystal deposition of periodontal ligament in patients with chronic kidney lead to bone absorption and destruction, further aggravated periodontal tissue damage, loosened teeth, and caused absorption of root outside, which resulted in tooth loss. Periodontal ligament and cementum injury and damage gradually increasedwith the progression of chronic kidney disease. Patients should undergo dental and kidney disease consultation before treatment of oral diseasesto determine the state of the disease, treatment options, complications, and the best treatment time. During the treatment of oral diseases, drugs for kidney damage, such as aminoglycosides and tetracyclines, should be avoided. For hemodialysis patients, treatment of oral diseases should be selected in non-dialysis sessionto reduce the risk of bleeding. Any optional treatment of oral diseases should be avoided in the first six months of renal transplant recipients.

    Primary failure of eruption(PFE) is a rare condition in which tooth eruption failure occurs without obvious local or systemic causes.PFE can also be caused by a disturbance in the eruption mechanism. This condition often affects permanent molars and causes a severe posterior openbite. PFE diagnosis and treatment remain confronted by difficulties. This article reviews the PFE clinical manifestations, etiology, clinical diagnosis, and treatment modalities.
