口腔颌面部肿瘤患者游离皮瓣修复术的并发症预防及处理

  • 作者简介:李春洁，口腔医学博士、教授、博士研究生导师、四川大学华西口腔医院头颈肿瘤外科主任，香江学者、四川省学术和技术带头人后备人选、四川省卫生健康委员会第十四批学术和技术带头人后备人选、国际牙医师学院院士、四川省青年文明号号长。任中华口腔医学会口腔颌面修复专业委员会及口腔颌面头颈肿瘤专业委员会常务委员，中国卫生信息与健康医疗大数据协会口腔健康大数据联合体副主任委员，四川省口腔医学会理事，四川省口腔医学会口腔颌面外科专业委员会、口腔遗传病专业委员会、口腔颌面修复专业委员会、四川省抗癌协会头颈肿瘤专业委员会、四川省医学会循证医学专业委员会常务委员。主持国家自然科学基金3项，作为子课题负责人参与国家自然科学基金重点项目1项，另主持各级基金8项。发表研究论文130余篇，以第一作者或通信作者发表SCI数据库收录论文75篇。研究生教材《循证口腔医学》第三版主编，《口腔颌面部肿瘤就医指南》副主编，参编多本专著。任《口腔颌面外科杂志》编委，多本中英文期刊审稿专家，国家自然科学基金函评专家。获得邱蔚六口腔颌面外科希望奖一等奖，2017年全国口腔医学青年教授教学比赛一等奖第一名，第二届中日口腔颌面外科联合会议青年医师比赛第一名。 毕小琴，四川大学华西口腔医院主任护师，医学博士，硕士生导师。四川省卫生健康委员会学术技术带头人。任中华护理学会口腔护理专业委员会副主任委员、中华口腔医学会口腔护理专业委员会常务委员、四川省护理学会口腔护理专业委员会主任委员、四川省口腔医学会口腔护理专业委员会主任委员。是教育部全国研究生教育评估监测专家、国家卫生健康委员会医药标准评审专家、《中华护理杂志》《中华现代护理杂志》等编委/审稿专家。发表论文100余篇，其中SCI数据库收录4篇，Medline数据库收录19篇；出版专著/规划教材31部，其中主编9部，副主编9部。负责主持国家卫生健康委员会/省科技厅/省医学会/省卫生健康委员会等课题/项目16项，负责国家继续教育项目2项。荣获全国教育教学成果大赛一等奖、全国优质护理先进个人、省医学科技进步三等奖等。

摘要：

快速康复理念对于口腔颌面部肿瘤患者十分必要，特别是针对手术并发症较多的接受了游离皮瓣修复术的患者尤其重要。口腔颌面部肿瘤患者游离皮瓣修复术的快速康复中，手术并发症的预防及处理是其中的重要内容。本文结合部分文献回顾及笔者在临床实践中的经验进行总结，包括气道管理、出血控制、术后感染预防、游离皮瓣修复术相关的手术并发症预防以及口腔颌面部功能相关并发症的预防和治疗，供相关医务人员参考，以期提高患者术后恢复速度，加强快速康复理念在口腔颌面部肿瘤患者游离皮瓣修复术中的应用。

Abstract:

Enhanced recovery is important for head and neck tumor patients, especially for those with free flap reconstruction which have numerous complications after surgery. Prevention and treatment of postoperative complications an important part for enhanced recovery after free flap reconstruction of head and neck tumor patients. In this article, we summarized the experiences in some published articles and authors’ clinical practice for the reference of related doctors, including airway management, hemorrhage control, prevention of postoperative infection, prevention of complications related to free flap reconstruction and functional complications prevention in oral and maxillofacial surgery, which could promote the application of enhanced recovery in free flap reconstructive surgery for oral and maxillofacial tumor patients.

口底肌群的悬吊A：因下颌骨颏部的节段切除导致附着于颏嵴的部分口底肌群失去附着，可能会使舌骨所维持的咽腔塌陷，导致通气受阻。B、C：将失去附着的口底肌群悬吊于下颌骨上，舌骨也因此被悬吊起来，能够尽可能避免术后出现气道塌陷。"
