The receptive and expressive language abilities in children with nonsyndromic cleft palate

摘要: 目的 观察非综合征性腭裂(NSCP)儿童的语言理解与表达能力与同龄正常儿童相比是否存在差异,并分析可能的发生机制。方法 对54例3~8岁居住于城市地区的NSCP儿童,按养育者受教育程度,以初中毕业为标准分为2组,养育者受教育程度初中毕业及以上的儿童记为NSCP-A组,养育者受教育程度初中毕业以下的儿童记为NSCP-B组,分别与相应的正常儿童(匹配年龄、性别、居住地和养育者受教育水平,记为正常对照A、B组),进行标准化语言能力测试(包括整体语言、语言理解、语言表达、语义和句法5个项目),观察NSCP组与正常对照组的测试结果是否存在差异。结果 NSCP组与正常对照组间的标准化语言能力测试得分的差异均有统计学意义(P<0.05);NSCP组在语言表达和句法应用能力项目上的得分均低于正常对照组;NSCP-B组在句法项目上的得分均值低于常模正常值。结论 与正常儿童相比,NSCP儿童对复杂语言结构的分辨和运用能力相对不足;养育者的受教育水平对患儿的语言能力有明显影响,养育者较高的受教育水平可以有效地提高NSCP患儿的语言表达和句法能力。因此,对NSCP儿童的序列治疗,应重视对患儿复杂语言结构运用能力的培养,以提高该群体的整体预后水平。

Abstract: Objective To observe the language skills in children with non-syndromic cleft palate(NSCP) in urban area. Methods Standardized diagnostic language assessments were conducted in 54 children with NSCP who lived in urban area. Subjects with the educational level of the guardian/caregiver equals grade nine and above were defined as subgroup NSCP-A, and those with the educational level of the guardian/caregiver lower than grade nine were defined as subgroup NSCP-B. Total language ability, receptive and expressive, semantic and syntax of the subjects were scored. Results were analyzed and compared to the sex/age matched normal control groups. Results Children in NSCP groups were found to be scored significantly(P<0.05) lower in expressive language and syntax tests than the normal control group, respectively. The syntax scores in subgroup NSCP-B children with NSCP were found below the normal range, implied that lower caregiver’s educational level could cause more negative impact on the syntax ability of children with NSCP. Con-clusion Children with NSCP could have impaired expre-ssive language and syntax skills and their ability for pro-cessing and using complex language structures could be affected by the educational level of the caregivers. Findings of the present study indicated that early assessment and interventions for language skills in children with NSCP should be conducted as components of the team approach.
