正中关系的可重复性考察及对其历史和发展的考量

Repeatability of centric relation registration methods and their effect on concept development

摘要:

正中关系是口腔医学界争议极大的一个概念,争论的焦点集中在髁突在关节窝内的空间位置关系。而关于该空间位置的争议,对临床工作的指导意义不大;使用时应回到正中关系概念的本源,即用于建立健康、稳定且功能良好的咬合关系的颌位。随着口颌系统功能检查手段与技术的日趋完善,以一个可重复的颌位为参考起点,可通过不断试错的方式,逐步寻找到该最适建颌位。这样可避免现有的各种正中关系记录法可重复性低且技术敏感度高的缺点。笔者通过改良现有的“下颌参考位”记录法,为这一过程找到一个可重复性高、操作简便易行的参考点获取办法,可获得良好的临床效果。

Abstract:

Centric relation (CR) is the most controversial concept in dentistry. The spatial relationship of the condyle within the fossae has been widely explored, however the author believes that studies should focus on the origin of the concept, which is, a favorable mandible position for developing occlusion that can maximize the function and health of the orofacial system. With numerous techniques to test mandible functions, an ideal CR can be found via a trial and error process, starting from a suitable and repeatable reference. The author developed a highly repeatable and convenient way to record the reference mandible position with favourable clinical outcomes and better repeatability than conventional highly skill sensitive methods by modifying the registration method of a previous reference position.
