两类不同金属烤瓷冠的成本-效果分析

摘要：

目的从卫生经济学的角度对非贵金属合金烤瓷冠和高贵金属合金烤瓷冠治疗牙列缺损的成本-效果进行评价。方法利用CNKI 数据库、CBM 数据库、VP 数据库检索符合条件的中文文献，对所筛选出的数据行 meta分析，比较其治疗后的临床效果并进行卫生经济学评价。结果12 篇文献符合纳入标准，非贵金属组相对于高贵金属组而言，修复后牙龈炎、颈缘染色、崩瓷、边缘适合性差的发生率差异均有统计学意义（P<0.05）。结局指标分别为牙龈炎、颈缘染色、瓷损坏、适合性差时，高贵金属组的成本-效果分析值分别较非贵金属组高 1 645.38 元、1 588.79 元、1 524.79 元、2 201.95 元。结论高贵金属烤瓷冠在临床效果和生物安全性上优于非贵金属烤瓷冠，非贵金属烤瓷冠在卫生经济学角度优于高贵金属烤瓷冠。

Abstract:

Objective To determine the effects, in economic aspect of clinical effects by porcelain fused-to-metal restoration made from Ni-Cr alloy or precious alloy during tooth defect or denture defect therapy. Methods A search for the objective articles was performed from the following database: CNKI, CBM and VP. RevMan software was applied to conduct meta-analysis. The clinical effects and economical analysis were studied. Results Twelve articles met the final selection criteria. The significant differences were found in the marginal color, gingivitis, damage, marginal fit the C/E of the precious alloy group was respectively 1 645.38 yuan, 1 588.79 yuan, 1 524.79 yuan, 2 201.95 yuan more than that of the Ni-Cr alloy group. Conclusion In clinical effects and biological safety aspect, the precious alloy is superior to Ni-Cr alloy. However, in economical aspect, the Ni-Cr alloy is to superior previous alloy.
