慢性牙周炎患者种植修复后种植体牙周指数及龈沟液炎性因子水平的变化研究

Changes on periodontal index and inflammatory factor levels in gingival crevicular fluid after implant restoration in patients with chronic periodontitis

摘要: 目的 研究慢性牙周炎患者种植修复后种植体牙周指数及龈沟液炎性因子水平的变化。方法 分别选取40例2015年6月—2017年6月于西南医科大学附属口腔医院修复科行种植修复的慢性牙周炎及牙周健康牙缺失患者为研究组及对照组,比较2组患者种植修复1、3、6个月种植体探诊深度(PD)、改良菌斑指数(mPLI)、牙龈乳头指数(PIS)、改良龈沟出血指数(mSBI)、边缘骨吸收量,并检测2组种植体周围龈沟液中白细胞介素(IL)- 1β、IL-6及IL-8水平。结果 2组种植体存留率均为100%,研究组种植修复后3个月开始,种植体周围龈沟液中IL-1β、IL-6及IL-8水平较治疗后1个月明显升高并高于对照组种植修复后3个月水平(P<0.05,P<0.01);种植修复后6个月,研究组PD、mPLI、mSBI及边缘骨吸收量较治疗后1个月明显升高且高于对照组种植修复后6个月水平(P<0.05,P<0.01),2组患者PIS比较差异无统计学意义(P>0.05)。结论 慢性牙周炎患者种植修复后近期疗效理想,但较牙周健康者有更高的种植体周围炎的患病风险。

Abstract: Objective This work aims to determine the changes in periodontal index and inflammatory factor levels in gingival crevicular fluid after implant restoration in patients with chronic periodontitis.Methods A total of 40 patients with tooth-loss in chronic periodontitis and healthy periodontium were selected as the research and control groups, respectively, and were treated with implant restoration from June 2015 to June 2017. Probing depth (PD), modification plaque index (mPLI), papilla index score (PIS), modification sulcus bleeding index (mSBI) and marginal bone loss volume were detected and compared at 1, 3, and 6 months after implant restoration between the two groups. The interleukin (IL)-1β, IL-6, and IL-8 levels in gingival crevicular fluid of two groups were detected and compared.Results The implant survival rate after implant restoration for the two groups were both 100%. Three months after implant restoration, IL-1β, IL-6, and IL-8 levels in the research group were significantly increased as opposed to the levels 1 month after implant restoration; and the mentioned levels in the research group are also higher than that of 3 months after implant restoration in control group (P<0.05, P<0.01). Six months after implant restoration, the PD, mPLI, mSBI and marginal bone loss volume for the research group increased compared with that at 1 month after implant restoration; the mentioned results in the research group are significantly higher than that of 6 months after implant restoration in control group (P<0.05, P<0.01). The PIS of the two groups showed no statistically significance (P>0.05).Conclusion The short-term effect of implant restoration in chronic periodontitis patients is ideal, but the risk of peri-implantitis is higher on patients with healthy periodontium compared with the other group.
